What retatrutide is
Retatrutide is a once-weekly, single-molecule peptide that activates three receptors: glucose-dependent insulinotropic polypeptide (GIP), glucagon-like peptide-1 (GLP-1) and glucagon2. Tirzepatide acts on the first two; semaglutide on GLP-1 alone. In trials so far, the added glucagon activity has been studied for its effects on weight, glucose and liver fat345. Eli Lilly describes retatrutide as an "investigational molecule" that is not approved for sale or for human use, and says it is legally available only to participants in its clinical trials12.
The phase 2 and phase 3 evidence
The phase 2 obesity trial, published in the New England Journal of Medicine in 2023, enrolled 338 adults. At 48 weeks, mean weight change ranged from −8.7% at the lowest dose to −24.2% at the highest, against −2.1% with placebo; gastrointestinal events were the most common adverse effects, and the authors reported dose-dependent increases in heart rate3. A phase 2 trial in 281 people with type 2 diabetes reported HbA1c reductions of up to about 2 percentage points at 24 weeks4. A substudy in metabolic dysfunction-associated steatotic liver disease found mean relative reductions in liver fat of up to 82.4% at 24 weeks, with liver fat normalising in up to 86% of participants on the higher doses5.
Phase 3 began reporting in 2026. In TRIUMPH-1, 2,339 adults with obesity or overweight were randomized to three retatrutide doses or placebo; mean weight reductions at 80 weeks were 19.0%, 25.9% and 28.3%, versus 2.2% with placebo1. Discontinuation for adverse events was about 4% to 11% on retatrutide versus about 4.9% on placebo, and dysesthesia, an abnormal skin sensation, was reported in about 5% to 12.5% of retatrutide participants versus 0.9% on placebo1. In July 2026 Lilly reported TRIUMPH-2, in adults with type 2 diabetes and obesity or overweight (weight loss up to 20.8% versus 4.0% at 80 weeks), and TRIUMPH-3, in severe obesity with established cardiovascular disease (up to 22.6% versus 3.2%)2. These phase 3 figures are taken from Lilly's press releases12.
The grey market and the US enforcement record
Strong trial headlines and no approved product have created a grey market. Public Citizen reported in June 2026 that the FDA has sent 14 warning letters since 2024 to companies selling retatrutide, that 11 of those companies were still advertising retatrutide or other unapproved peptide drugs as of May 2026 and eight were still selling retatrutide itself, and that the FDA's adverse event monitoring system listed 14 hospitalizations linked to retatrutide6. Its research director, Peter Whoriskey, said: "The era of snake oil and quack medicine is back"6. The group also found that hundreds of social media influencers act as sales affiliates for these sellers6.
No published laboratory study has yet tested retatrutide bought online in the way researchers have tested semaglutide. For semaglutide, test purchases from illegal online pharmacies found vials with 7.7% to 14.37% purity against a 99% label claim and endotoxin in every sample11. There is no reason to assume an unapproved molecule with no licensed manufacturer is handled better.
What Bahamian law says
The Bahamas has no product-by-product approval to keep an unapproved drug off the market. The Chief Pharmacist at the Ministry of Health and Wellness, Dr Gina Archer, told The Tribune in February 2026 that "The Bahamas currently does not have a national drug registration system"8, and a pharmacist described the country as "a non-registration market" without batch testing9. The Pharmacy Act, 2009 still sets limits. Section 27 requires any drug manufactured, sold, imported or exported to meet a prescribed international standard such as the British, United States or European Pharmacopoeia, with fines of up to $20,0007. Section 25(3) states that "No person shall establish or engage in the practise of internet pharmacy in any form"7. An investigational molecule sold through websites runs into both provisions.
The demand exists. The country's only full-time endocrinologist told The Tribune in September 2026 that demand for approved GLP-1 drugs is "significantly and steadily increasing"; with monthly costs of $250 to more than $1,500, the paper reported a growing market for their "unregulated acquisition"10. The World Health Organization's December 2025 obesity guideline covers liraglutide, semaglutide and tirzepatide; retatrutide is not among the medicines it evaluated12. PAHO's February 2026 alert on GLP-1 misuse asks prescribers to keep use within approved indications, with individual assessment and continuous monitoring13.
